Who qualifies for PACE in Kentucky?

PACE (Program of All-Inclusive Care for the Elderly, 907 KAR 3:250) serves people 55 and older who need nursing-facility level of care, live in a PACE organization's service area, and can live safely in the community with PACE's help.

Services are paid by Medicare, Medicaid or privately, depending on the enrollee. Most enrollees are dual-eligible for Medicare and Medicaid, in which case PACE typically has no premium for them.

What does PACE cover?

A PACE organization becomes the enrollee's whole care system: primary care, social services, physical and occupational therapy, personal care and supportive services, nutrition counseling, recreational therapy and meals, delivered at the PACE center, at home or in other facilities.

If an enrollee needs something the PACE organization does not provide directly, it contracts for it. The trade-off is that enrollees use the PACE organization's doctors and network. For a Louisville family juggling several specialists, that consolidation is the point; for someone attached to a long-time physician outside the network, it can be a reason to say no.

How do Louisville families apply for PACE?

Contact the PACE organization that serves your ZIP code directly; DMS advises having the person's name, date of birth, Social Security card, diagnosis and medical information ready, and DAIL answers PACE questions toll-free at (888) 804-0884.

If Medicaid will be part of the payment, start the Medicaid application with DCBS in parallel. KIPDA's ADRC at (502) 266-5571 can explain how PACE compares with the HCB waiver for a particular situation.
