A parent is being discharged from a Louisville hospital

What to do when a case manager gives you days, not weeks.

Quick answer

Ask the hospital case manager for the written discharge plan, confirm in writing whether your parent was admitted as an inpatient or kept under observation, and ask which rehab facilities or care settings have actually accepted the referral.

What should I do first when a Louisville hospital says my parent is being discharged?

Ask the hospital case manager for the written discharge plan, confirm in writing whether your parent was admitted as an inpatient or kept under observation, and ask which rehab facilities or care settings have actually accepted the referral.

The Louisville hospitals on Kentucky's OIG hospital roster include UofL Health's Jewish, Mary and Elizabeth and University of Louisville hospitals, Norton Hospital, Norton Audubon, Norton Brownsboro, Norton West Louisville and Norton Women's and Children's, and Baptist Health Louisville, with Baptist Health La Grange, UofL Health Shelbyville and UofL South Hospital in Shepherdsville serving the outer counties. Each has case managers whose job is a safe discharge; use them.

Why does inpatient versus observation status matter for Medicare rehab?

Original Medicare pays for skilled nursing facility care only after a qualifying inpatient hospital stay; nights spent under observation status do not count, however many there were, so a parent who was never formally admitted may have to pay privately for rehab.

Medicare Advantage plans set their own rules and usually require prior authorization for a skilled nursing stay, so call the plan the same day. If status is observation, ask the physician whether inpatient admission is medically appropriate.

How do I check a rehab facility or care home quickly during a discharge?

For a nursing facility, open its CMS Care Compare record or our facility page for its overall rating, latest inspection date and deficiencies; for assisted living or a personal care home, confirm the license type and expiration on OIG's monthly directory.

Five minutes is enough to avoid the worst mistakes: an expired license, a Special Focus Facility designation, or an assisted living license that cannot administer the new medications on the discharge list (a social-model ALC cannot give injections, eye drops or nebulizer treatments).

Can the discharge date be pushed back?

Often, yes: a discharge date is less fixed than it sounds when no safe plan exists, and Medicare patients have the right to appeal a discharge they believe is premature through the notice the hospital must give them.

Ask what services can start at home immediately (Medicare home health can begin within days), whether KIPDA's ADRC at (502) 266-5571 can arrange short-term supports, and whether a respite stay is possible while a longer-term plan comes together.
